This book focuses on identification and resolution of drug-related problems (DRPs) utilizing a systematic approach. Each chapter begins with the 'description of the patient', i.e. demographics, present complaints, past medical history, family history, social history, allergies, medication history, relevant vitals and laboratory values. This is followed by the 'DRP assessment' consisting of checking for the presence of any DRP class and providing solutions of the identified DRPs according to the most recent pharmacotherapy knowledge. It includes specific chapters on individual cases of several disease states encountered in the hospital setting, namely: sepsis, atrial fibrillation, heart failure, hyperlipidemia, hypertension, hypothyroidism, diabetes, epilepsy, ischemic stroke, chronic obstructive pulmonary disease, chronic kidney disease, urinary tract infection, pancreas cancer, and acute myeloid leukemia.
