La tesis que mantiene el autor es que no se trata tanto de «curarse» de la enfermedad, pues algunas de las que aquí aparecen, como el autismo, el síndrome de Tourette, la acromatopsia, no admiten el concepto de curación, sino de aprender a vivir con ella, a pesar de lo grotescos que puedan ser algunos síntomas de lo diferente que pueda sentirse el paciente respecto a los así denominados normales. Todos estos pacientes encuentran en el doctor Sacks no sólo un médico atento, intuitivo y perspicaz, sino a alguien que comparte sus vidas y aplica el método empático de investigación que G. K. Chesterton presentara a través de su personaje más famoso, el padre Brown.
